We no longer provide a consent form ahead of time. Instead, you will complete a form when you arrive to the shelter. 

The discharge information is everything you will need to know for caring for your cat(s) after they go through our program. We will be happy to answer any questions that you have when you drop off or pick up your cat(s) however we cannot give medical advice.
